To complete an injury claim form, download the ACC45 User Guide which will take you through the steps involved.
The ACC45 form has been specifically designed and developed to make it easier to do business with ACC by providing a user friendly form that’s quick to complete.
It is delivered from the web but leaves the claim information in the Patient Management system.
Work and Income introduced e-Lodgement of Work Capacity medical certificates on 10 June 2015. This means GP practices can submit these medical certificates electronically rather than printing and signing them.
Importantly it means patients won’t have to make a trip to Work and Income to drop off their medical certificate.
e-Lodgement significantly improves the transfer of relevant information between practitioners and Work and Income. GP practices also benefit from the improved automation and simplified work flow e-Lodgement allows.
If your practice doesn’t currently have access to HealthLink’s online forms please contact us to arrange this free system upgrade.
For more information on using e-Lodgement of Work Capacity medical certificates please download a Quick Start Guide below.
